Part of navigating the medical system during times of severe and sudden illness is to document your symptoms, doctor visits, medications, steps and general research notes. New doctors, nurses and medical professionals will continuously ask patients and caregivers for the same information over and over again. Keeping all this information in order and accurate can be exhausting for patients and caregivers. I created these documents in the Fall of 2012 when I was beginning my own battle with unfamiliar symptoms and an eventual and ongoing battle with eye/brain tumors and chemo/radiation treatments.
I hope these documents can be helpful to you. Please adapt them however you see fit. Download the Word Documents below and save them on your computer. If you want to have them in your Google Docs then upload them there first and then start to fill them in there.

- I use these documents as Google Docs primarily so that I can reference them digitally and from my phone, just in case I forget to bring a paper print out with me to a given doctors appointment or rush to the ER.
- I update these documents after every doctor visit and with every change of medication.
- I print out at least 2 copies to have on hand at any time and I travel with a smaller version in my toiletry bag and wallet.

- BRB! is a deck of cards, each with a simple prompt for self care and regenerative activities that contribute to your own sustainability in social justice work. The work we do for social justice and transformation is hard. It’s hard on our bodies, our mental health, and our spirits. As the popular movement maxim goes, this work is a marathon, not a sprint. Draw a card, and take a little time to step away, and take action that fills the well, so you can "be right back" for yourself and the movement.
- Story Detours are simple instructions meant to help in moving creative strategic thinking off our well-worn tracks. Having trouble with your strategy? Feel like you've been telling the same story for too long, without enough impact? Have a strategy, but stuck on how to tell the story?
Draw a Detour card when you’re stuck crafting a story-based strategy, and follow the instructions to experiment and give your imagination a different route to take. - Staring at a blank page can be overwhelming, even for experienced change-makers planning a creative action. Get the creativity flowing by getting out of analysis mode and just prototyping an intervention, using the Picture This! deck. Each card indicates three constraints for you to follow in designing an action or intervention:
A Point of Intervention – where you will intervene
A Medium – what form the intervention must take
An Emotion – the emotional reaction your intervention is intended to evoke in the audience
